COMPETITION: Name of Competition
DATE: Date of Competition
TIME: Tee Time
PLAYER DETAILS: Full Name (Not just initials)
HANDICAP INDEX: Can be found on IG app, MyEG app or on the screen in the bar
COURSE HANDICAP: Found on the relevant Course Handicap Table on the Pro Shop wall
PLAYING HANDICAP: Calculated as Course Handicap x Handicap Allowance
TEES USED: Tick White/Yellow/Red box or write on card if playing Forward Tees (Seniors)
SCORES: Enter Gross Scores and Totals (and Bogey/Stableford Scores if applicable)
MARKER’S SIGNATURE: Signature of Marker
PLAYER’S SIGNATURE: Signature of Player
